Death Certificate: Meghalaya
Information
regarding all births, deaths and still births occurring within the jurisdiction
of the Shillong Municipal Board has to be given in the prescribed form to the
birth and death department within 21 days from the date of birth or still
birth; and
The
prescribed forms for the purpose are available with the health officer of the
birth and death department free of cost.
Documents
1. If the death occurred
at home, the death certificate from the locality headman should be enclosed.
2. If the death occurred
in the hospital, the death certificate from the hospital should be enclosed.
